Raises Testosterone While Improving Sperm Count

This is the clinical outcome that makes enclomiphene uniquely valuable — and fundamentally different from every other testosterone intervention. Traditional testosterone therapy raises T but suppresses sperm production. Enclomiphene raises T while simultaneously improving sperm parameters.

In Phase III clinical trials, men on enclomiphene maintained sperm concentrations in the normal range throughout treatment. Men on testosterone gel — at the same testosterone levels — showed marked reduction in spermatogenesis. The difference was not marginal. It was decisive.

BJU International, 2016 · Phase III RCT
Kim et al. — "Oral enclomiphene citrate raises testosterone and preserves sperm counts in obese hypogonadal men, unlike topical testosterone"
Enclomiphene maintained sperm concentrations in the normal range; testosterone gel caused marked reduction in spermatogenesis in the same study.
Journal of Sexual Medicine, 2013 · Phase IIB RCT
Kaminetsky et al. — "Oral enclomiphene citrate stimulates the endogenous production of testosterone and sperm counts"
Sperm counts elevated in 7 of 7 men at 3 months and 6 of 6 men at 6 months, with concentrations in the 75–334 million/mL range.
Archives of Endocrinology & Metabolism, 2025 · Meta-Analysis of 10 RCTs
Hohl et al. — "Clomiphene or enclomiphene citrate for the treatment of male hypogonadism: systematic review and meta-analysis" (n=819)
SERM therapy showed significant improvement in sperm concentration parameters compared with testosterone gel across all included trials.
7/7
Men improved sperm count at 3 months
In Phase IIB clinical trials, every single patient on enclomiphene showed elevated sperm counts at the 3-month mark.
75–334M
Sperm concentration range achieved
Per milliliter. Normal is considered above 15 million. These patients achieved 5 to 22x normal levels.
0
Sperm suppression on enclomiphene
Unlike testosterone replacement, enclomiphene produced no reduction in spermatogenesis across any trial.

Restores Testosterone to Optimal Levels Naturally

Enclomiphene does not replace testosterone. It removes the brake. By blocking estrogen receptors in the hypothalamus, it restores the pituitary signal that tells the testes to produce testosterone — the way the body was designed to work.

In clinical trials, enclomiphene achieved testosterone levels equivalent to those produced by prescription testosterone gel — without any of the suppressive effects on the HPG axis. The same results. The natural pathway preserved.

Most men see testosterone levels begin normalizing within 2 to 4 weeks of starting the protocol. Full clinical benefit develops over 8 to 12 weeks of consistent use.

Fertility & Sterility, 2014 · Phase IIB RCT
Wiehle et al. — "Enclomiphene citrate stimulates testosterone production while preventing oligospermia"
Treatment resulted in increased morning serum testosterone, LH and FSH levels similar to those obtained with topical testosterone gel — while preserving sperm counts.
Journal of Men's Health, 2014
Wiehle et al. — "Enclomiphene citrate stimulates serum testosterone in men with low testosterone within 14 days"
Significant rises in total testosterone observed within two weeks of initiating oral therapy at both 12.5mg and 25mg doses.
545
Average testosterone ng/dL achieved at 6 months
Compared to an average baseline of 165 ng/dL in patients who had come off testosterone therapy. Mid-range optimal is 600–800 ng/dL.
14
Days to first measurable improvement
Significant testosterone increases were observed within 14 days of starting the protocol in Phase II trials.
273
Average ng/dL increase over placebo
2025 meta-analysis of 819 patients found a mean difference of 273.76 ng/dL versus placebo. Clinically significant in every trial.

Preserves the Natural Hormonal Axis — Fully Reversible

When a man takes exogenous testosterone, his brain detects excess testosterone in the bloodstream and shuts down its own production signals. The testes stop receiving LH. Testosterone production stops. Sperm production stops. This shutdown can persist for months to years after stopping treatment.

Enclomiphene works in the opposite direction. It amplifies the natural signal. LH and FSH levels rise. The testes respond. When the protocol is stopped, the body's own system continues — there is no rebound suppression, no recovery period, no dependency.

Expert Opinion on Pharmacotherapy, 2016
Rodriguez, Pastuszak & Lipshultz — "Enclomiphene citrate for the treatment of secondary male hypogonadism"
Enclomiphene restores testosterone through the natural HPG axis without the fertility suppression associated with testosterone replacement therapy.
Review of Urology, 2016
Rambhatla, Mills & Rajfer — "The role of estrogen modulators in male hypogonadism and infertility"
SERMs like enclomiphene maintain or improve spermatogenesis while elevating testosterone, making them the preferred option for men who wish to preserve fertility.
0
HPG axis suppression
Unlike testosterone therapy, enclomiphene produces no suppression of the hypothalamic-pituitary-gonadal axis. The body's own production continues throughout.
100%
Reversible upon discontinuation
Because the body's natural system remains active, there is no recovery period required when stopping. Natural function continues where it left off.

He Will Feel the Difference — Energy, Drive, Clarity

Improved fertility is the goal. But optimized testosterone delivers more than a better sperm count. Men on enclomiphene consistently report meaningful improvements in energy, focus, mood stability, and libido — all downstream effects of restored testosterone.

These are not side effects. They are the other half of what optimal testosterone does for a man in his 30s and 40s. The fertility protocol and the optimization protocol are the same protocol.

Journal of Sexual Medicine, 2024 · Baylor College of Medicine
Kassab, Saffati, Lipshultz & Khera — "Safety and efficacy of enclomiphene compared to clomiphene for hypogonadal men"
Patients on enclomiphene reported significantly fewer side effects than clomiphene, including fewer mood changes and decreased libido — suggesting a cleaner hormonal profile.
Archives of Endocrinology & Metabolism, 2025
Hohl et al. — Meta-analysis of 10 randomized controlled trials (n=819)
No significant adverse events reported across included studies. No substantial changes in fasting glucose, insulin, or BMI — confirming favorable safety profile.
2–4
Weeks to first noticeable energy improvement
Most patients begin noticing improvements in energy, mood, and focus within 2 to 4 weeks of starting the protocol.
819
Patients across 10 trials with no major adverse events
The 2025 meta-analysis confirmed a favorable safety profile with no substantial adverse events across nearly 1,000 trial participants.

Common questions

What You Need to Know

Is enclomiphene FDA approved? +
Enclomiphene citrate is not FDA approved as a finished drug product. It is prescribed off-label through licensed physicians and compounded by licensed 503A compounding pharmacies. It is legally prescribable in all 50 states under physician discretion. All Viro Health protocols are issued by licensed physicians and compounded by licensed pharmacies operating within applicable regulations.
Will this actually improve his sperm count? +
In men with secondary hypogonadism — where the problem is in the signaling pathway between the brain and testes, not in the testes themselves — clinical trials have shown consistent improvement in sperm parameters. The 2025 meta-analysis of 819 patients confirmed significant improvement in sperm concentration compared to testosterone gel. However, enclomiphene is not effective for primary hypogonadism where the testes themselves cannot respond. The physician assessment determines which type is present before prescribing.
How is this different from clomiphene or Clomid? +
Clomiphene contains two isomers: enclomiphene and zuclomiphene. Zuclomiphene has estrogenic effects that can cause mood changes, visual disturbances, and other side effects. Enclomiphene isolates only the beneficial anti-estrogenic component. Clinical research from Baylor College of Medicine found enclomiphene to be equally effective for testosterone restoration with significantly fewer side effects than clomiphene.
How long before we see results in fertility? +
Testosterone levels typically begin normalizing within 2 to 4 weeks. Sperm parameters — count, motility, and morphology — take longer to improve because the sperm production cycle is approximately 72 to 90 days. Most men see meaningful sperm improvement within 3 to 6 months of consistent protocol adherence. Testosterone-related benefits like energy, mood, and libido are typically felt within the first month.
Does he need to get labs first? +
Baseline labs are strongly recommended and included as an add-on at checkout. The physician needs to confirm secondary hypogonadism — not primary — before prescribing enclomiphene. Labs also establish a baseline for monitoring progress. We offer a Hormone Baseline Panel for $89 that covers Total T, Free T, LH, FSH, and Prolactin. The physician reviews results before issuing the prescription.
Can he cancel or stop at any time? +
Yes. No contracts, no cancellation fees. Cancel through the patient portal or by contacting support at any time. Because enclomiphene works by restoring the natural hormonal signal rather than replacing testosterone, there is no required taper or recovery protocol when stopping. The body's natural system resumes.
